The game introduces new enemy types including armored knights and dark dwarves, while expanding the bestiary of rideable creatures with two-headed dragons and giant scorpions that offer unique combat advantages.

While criticized for being too similar to its predecessor, Golden Axe II was praised for its smoother animation, larger character sprites, and the introduction of branching paths that increase replayability.

The game's soundtrack, composed by the legendary Yuzo Koshiro (Streets of Rage), features heavier use of the Genesis sound chip for more intense battle music that became iconic among 16-bit beat 'em ups.
